Most patients with liver involvement are asymptomatic. Pruritus, jaundice and pain in the right upper quadrant of the abdomen are the major symptoms of hepatic sarcoidosis.

Long-standing hepatic sarcoidosis can trigger complications like portal hypertension and hepatic cirrhosis.

Not all cases of liver sarcoidosis require treatment. For symptomatic patients, first treatments include corticosteroids or ursodeoxycholic acid. Second line agents can include immunosuppressant agents. Severe cases require liver transplantation.
